TERMS & CONDITIONS Please remember that our terms and conditions are created for the safety of our passengers, employees, others and equipment. By making your reservation with GoldStar Executive Transportation Service of Arizona L.L.C. you agree to be bound by the following terms and conditions. Reservations Reservations for services must be made at least 24 hours in advance to guarantee availability. You can make a reservation either thru our web site (www.mygoldstaraz.com) or by contacting us directly at 480.699.5625 or Fax us at 602.277.3929. Requests for service will be acknowledged with an E-mail confirmation containing a confirmation number. Please reference this number in all correspondence with us. All confirmations must be activated by having a valid credit card on file. Your credit card will be used as a guarantee for payment of services. All reservations made less than 24 hours are subject to availability. All reservations require a 50% deposit which is non refundable but may be used for later travel A 20% gratuity will be included in your confirmation along with a 6% surface transportation fee plus any other applicable fees (Fuel surcharge, Deposits,) Reminder -The confirmation is not necessarily the total amount due, the actual amount will be calculated upon completion of the trip NOTE - Once you receive your confirmation, please review it for accuracy. GoldStar Executive Transportation will not be responsible for missing or incorrect information Cancellations - We understand your travel plans may change and other circumstances may arise. With that in mind, GoldStar Executive Transportation Service has a very simple cancellation policy. GoldStar Executive Transportation Service of Arizona L.L.C. must be notified of all cancellations. Cancellations can be submitted by either E-mail or by phone 480.699.5625. Upon canceling your service you will receive a cancellation reference number. Cancellations for services within the Phoenix metro area using Town cars or SUV's require a 2 hour minimum notification. Services using limousines or Buses require a 7 day minimum notification. For services outside the phoenix metro area we require 48 hour notice. Any cancellation of services of less than minimum notifications will be charged for the full amount of the agreement. Canceled flights are exempt from the cancellation policy, but not missed flights.
Rates GoldStar Executive Transportation Service has standardized point-to-point rates that provide flat rate pricing for travel to and from most destinations in Arizona. Rates for travel between locations in which a point-to-point rate does not exist will be billed at Goldstar's prevailing hourly rate. Customers are billed from the time the vehicle leaves Goldstar's facility (3400 East Sky Harbor Boulevard, Phoenix) until the time it returns to the facility, with a two-hour minimum. Rates quoted for service provided by GoldStar Executive Transportation are estimates based on the information given at the time of booking. Final charges will be assessed upon service completion based on the actual services provided. Additional charges that can be included: Additional Wait Time, Holiday Surcharge, Fuel Surcharge, Extra Stops, After Hours, Gratuity, Domestic Baggage-Claim Meet (upon request), and other fees. These additional charges, if incurred, will be itemized on the invoice. Hourly rates will be rounded off to the next half hour. All hourly rates include 20 miles of travel for each hour used, excess mileage will be calculated at $2.50 per mile. Rates are subject to increases during holidays and peak times. All published rates are subject to change without notice. Current rates are confirmed at the time the reservation is made. A 20% gratuity fee for the driver is added to all base rates. Additional Charges Scheduled pick-up time: A 10-minute courtesy is given on all transfers if you should go beyond that time, then wait time charges will commence back to the original scheduled pick-up time. Wait time will be calculated at our hourly rate. Airport arrivals are excluded from this policy. (See airport service) Wait time - Stops along the way (Point to point transfers only.) Stops en route will be assessed $10.00 per stop charge which includes 10 minutes of waiting time. Stops that change the normal route substantially to the final destination will be assesd additional mileage charges, at a rate of $3.00 per mile. Additional wait time will be calculated at our hourly rate.
Airport Service: Airport Arrival Instructions If we are transporting you to the airport, we recommend that you be at the airport at least one and one-half hours prior to your flight. Airlines routinely suggest that you arrive at the airport at least two hours before your flight. If we are meeting you at the airport, there are two options: Curbside Pick-up: After you have claimed your baggage, please call us immediately at 480-699-5625. Then proceed to the designated "Blue Diamond" area. Your reserved vehicle will be waiting on airport property, and will arrive at the "Blue Diamond" momentarily. There is no additional charge for curbside pick-ups. Please be advised that airport regulations require livery vehicle to remain in a holding lot until we have heard from the passenger, so it may take a few minutes for your car to reach you. Because of this, it is important that you are ready to leave when you call our dispatch. Meet & Greet service - Domestic Baggage Claim area: Arrangements for this service must be made in advance. The driver will meet you at the baggage claim area with a sign displaying your name. An additional charge of $25 will apply for the domestic baggage-claim meet. If you do not see your driver, DO NOT LEAVE. In the event of a mix-up (which would include last-minute gate changes, incorrect information on the terminal "arrivals" monitor or the terminal parking lot is full or closed), please contact us immediately at 480-699-5625. Goldstar will hold you responsible if you do not inform us about your travel status and will result in "no show" charges to be charged to your account. If you decide to leave, you are responsible for the full charge, including base rate, gratuity, meet/greet charges, parking fees, wait time, and applicable tolls and taxes in connection with the reservation. Grace Periods and Wait Time: Domestic Flight Grace Period and Wait Time: A 30-minute grace period is allowed following the flight arrival time which is posted on the airline web site or the airline information board. Wait time will be charged at $15 per 15-minute increments once the grace period has elapsed. If you need your driver to wait more than 45 minutes, please contact us immediately during this 30-minute grace period. Our drivers are instructed not to wait for more than 45 minutes if contact between you and the driver has not been made. If you fail to contact us within that time period, you will be responsible for the full amount that would be due plus one-hour wait time. International Flight Grace Period and Wait Time: A 45-minute grace period is allowed following the flight arrival time which is posted on the airline web site or the airline information board. Wait time will be charged at $15 per 15-minute increments once the grace period has elapsed. If you need your driver to wait more than one hour, please contact us immediately during this 45-minute grace period. Our drivers are instructed not to wait for more than one hour if contact between you and the driver has not been made. If you fail to contact us within that time period, you will be responsible for the full amount that would be due plus one-hour wait time.
Agreement & Liability . You agree that there will be no smoking or consumption of illegal drugs in GoldStar Executive Transportation vehicles. Anyone who smokes will be subject to a $100 charge and asked to extinguish the cigarette/cigar/pipe. The driver is authorized to terminate this agreement if our "no smoking" policy is ignored. No refunds will be given. . Drug use is not tolerated under any circumstances. The driver is authorized to terminate this agreement if there is drug use in the vehicle. No refunds will be given. . You agree that the passenger capacity of your reserved vehicle shall not be exceeded. . In case of any misconduct by you or a member of your party, the driver is authorized to terminate this agreement without any refunds. . You agree to hold GoldStar Executive Transportation harmless and not liable for any personal or material damages arising from the conduct of any member of your party. . You are responsible for any damages committed to Goldstar's vehicle by any member of your party during service, either willfully or accidentally. Any actions that cause damage to a Goldstar vehicle can result in an extra charge of a minimum $100, including loss or theft of property from the vehicle. . You are responsible for full payment of any overtime charges, beyond the original agreement. . Goldstar is not responsible for delays or inconveniences due to unknown or unforeseen mechanical failures or situations deemed as "Acts of God." . There is a $300 minimum charge for cleaning vehicle. . There will be no "popping out" of the sun roof, if your reserved vehicle has a sunroof. If any member of your party is a minor, the sunroof will remain locked, and a parent's telephone number must be given to the driver prior to the ride. Safety Concerns We carry many passengers for whom safety is our number one concern, including lone female travelers, and occasionally unaccompanied minors. We take your safety very seriously. As an extra security precaution, we can arrange for a password to be exchanged between the passenger and the driver. Additionally, if you are particularly comfortable with a specific driver, feel free to request that person as the driver. We will do all we can to accommodate your request. Seat belt Usage Arizona state law requires that all passengers use a seat belt. We strongly urge all passengers to abide by this law. Any member of your party who is a minor under the age of 5 MUST use a seat belt or if the minor is less than 40 pounds MUST be restrained in an approved child seat. Furnishing the child seat and buckling said seat into Goldstar's vehicle is the responsibility of the parent of guardian accompanying the minor. GoldStar Executive Transportation will provide a car seat at the rate of $10. Arrangements must be made prior to pick-up. All of our vehicles are non smoking Lost and Found GoldStar Executive Transportation assumes no responsibility for lost or damaged personal belongings that are left behind in any of our vehicles. We will make every attempt to return any found items in our vehicles. Please be aware of your personal property at all times Our Guarantee We at GoldStar Executive Transportation guarantee to do our best in transporting you to your destination on time. However, because of the nature of the transportation business and due to some situations that are beyond our control, we will not be held liable for claims resulting in such delays. We will always do our best to see that you arrive at your destination on time. Goldstar reserves the right to substitute the contracted vehicle with a comparable vehicle if circumstances warrant such substitution.
